ABSTRACT

Objective:To investigate clinicopathological and magnetic resonance imaging (MRI) characteristics of pediatric cardiac tumors.Methods:The clinical, pathological and MRI data of 7 patients with pediatric cardiac tumors confirmed by pathological examination in Children's Hospital of Chongqing Medical University from February 2012 to December 2016 were retrospectively analyzed.Results:There were 3 males and 4 females with first diagnosis age ranging from 1 month to 3 years. As for clinical presentation, most cases were featured with cardiac murmur and enlarged cardiac boundary; only 1 case had acute cerebral infarction, and 1 case did not show any abnormal performance. Pathological findings showed that 6 cases of benign tumors (including 2 cases of fibroma, 1 case of rhabdomyoma, 1 case of myxoma, 1 case of lipoma and 1 case of hemangioma), 1 case of malignant tumor (primitive neuroectodermal tumor of pericardium). MRI results showed that the signal intensity of malignant tumor was higher than that of normal myocardium in each sequence; significant differences were found in benign tumors; first-pass perfusion, cardiac cine image and late gadolinium enhancement were the most obvious.Conclusions:The clinical presentations of pediatric cardiac neoplasms are atypical. Each tumor type has pathognomonic pathological features. MRI has great advantages in the diagnosis and differential diagnosis of cardiac tumors especially for benign tumors.

ABSTRACT

ObjectiveEsophageal cancer is a common malignant tumor of the upper gastrointestinal tract, and has high incidence and mortality in China. Its incidence is increasing year by year, and survival rate is low, thus seriously threatening human life and health. To further explore the pathogenesis of esophageal cancer and its systematic and efficient diagnosis and treatment methods, the animal models of esophageal cancer was evaluated according to the animal model evaluation method previously established by our team based on the characteristics of its clinical symptoms of traditional Chinese and western medicine, and suggestions for model improvement were proposed. MethodThe existing animal models of esophageal cancer were summarized through China National Knowledge Infrastructure (CNKI) and Wanfang Data. The relevant indexes of the models were assigned, and their coincidence with the clinical diagnostic guidelines of traditional Chinese and western medicine for esophageal cancer was evaluated. ResultExcept the spontaneous animal model of esophageal cancer with high clinical coincidence adopted in few studies, the animal model induced by methylbenzylnitrosamine was in good agreement with the clinical data, which simulated the etiology and pathogenesis of esophageal cancer to a certain extent. The model partially reflected some indicators of clinical diagnosis in western medicine, and also indicated the body weight loss, purple nail and increased number of drinking in traditional Chinese medicine (TCM). However, there was still a lack of differentiation of TCM syndromes. ConclusionOn the basis of the original model, the animal model induced by methylbenzylnitrosamine and the mouse model of xenogeneic gastric wall transplantation of human esophageal cancer cells were applied, which were intervened with the factors of phlegm and qi mutual obstruction syndrome, blood stasis and phlegm stagnation syndrome, Yin deficiency and internal heat syndrome and Qi deficiency and yang weakness syndrome, and were distinguished to reflect the performance of TCM syndrome. The animal model of esophageal cancer combined with TCM syndrome was thus obtained, which embodied the clinical symptoms of esophageal cancer in TCM, and the characteristics of the animal model combined with TCM syndrome, and simulated the clinical practice of traditional Chinese and western medicine, providing an accurate pathological model carrier for basic research.

ABSTRACT

Aiming at the contradiction between the current oncology teaching model and the rapid development of modern oncology, this study proposes to use precision medicine principles, integrated medicine (MDT treatment) and problem-based, evidence-based CSCO guide learning in clinical oncology teaching. Through specific cases, students are instructed to study the guidelines, and cultivate students' concepts of evidence-based medicine, students' interest in participating in oncology teaching, and they can initially form treatment strategies. The prospects have been put forward from the establishment of a clinical oncology curriculum system, the construction of a single-disease diagnosis and treatment teaching environment and a team of teaching staff, the reinforcement of students' basic experiments and evidence-based clinical data cross-application learning, and the enhancement of humanistic quality education.

ABSTRACT

Chaihu Shugansan is a representative prescription for soothing the liver and relieving depression in traditional Chinese medicine(TCM). Chaihu Shugansan is from Jingyue Quanshu.Chaihu Shugansan has been widely used in clinic since Ming dynasty because of its exact curative effect.Chaihu Shugansan has the effect in soothing the liver and relieving depression through multiple components, multiple ways and multiple targets.At present, the mechanism of Chaihu Shugansan has been interpreted based on its multiple effect on neurotransmitters, cytokines, neuronal injury signal pathways, neuroendocrine, oxidative stress and so on.This paper summarizes the progress of clinical studies on source, pharmacology of single herbs and effective components of Chaihu Shugansan, in aspects of cardiovascular system, neuropsychiatric system, digestive system, reproduction-related systems, analyses and summarizes relevant literatures of Chaihu Shugansan published in recent years, and discusses about current development and shortcomings of Chaihu Shugansan, so as to provide ideas for the studies in the future.Beneficial explorations will be conducted in large-sample-size, standardized, clinical and experimental mutual verification.In proteomics research technology, network pharmacology, supramolecular theory of TCM and other fields, Chaihu Shugansan and liver depression syndrome will be organically connected from the microscopic perspective.And comprehensive studies will be conduct to TCM, metabonomics, proteomics, genomics and other disciplines, in order to clarify Chaihu Shugansan prescriptions, syndromes, therapeutics and its mechanism.

ABSTRACT

Objective@#In order to improve the accuracy of preoperative diagnosis, the value of MRI in the diagnosis and differential diagnosis of the mediastinal neuroganglionic tumors in children was explored.@*Methods@#A retrospective analysis of 41 cases of pathologically proven mediastinal neuroganglionic tumors[GN 19 cases, 6 cases of ganglion neuroblastoma (GNB), 16 cases of neuroblastoma (NB)]was conducted.The MRI characteristics and clinicopathological features were analyzed, including age, size, shape and boundary of tumor, MRI plain scan and enhanced signal characteristics, compared with postoperative pathology.@*Results@#The average age of the 19 cases of GN was 3.9 years, the maximum diameter of the tumor was 7.3 cm, 12 cases of tumor morphology, 18 cases of clear border, 11 cases of tumor interlinking with vertebral canal, 0 case with multi nodular fusion, 12 cases with lengthwise tumor, 11 cases with low signal in T1WI, 12 cases of uniform high signal in T2WI, and 2 cases with hemorrhagic necrotic cysts and cystic degeneration.In 6 cases of GNB and 16 cases of NB, the above-mentioned signs were 4.1 years old, 1.9 years old, 6.8 cm, 6.4cm, 2, 6; 5, 12; 3, 15; 2, 10; 3, 12; 0, 4; 3, 9; 2, 11; 1, 2; 5, 9; 0, 7, there were statistically significant differences in age of onset(F=4.145; P=0.024), relationship between tumors and vertebral canal(P=0.023), hemorrhagic necrosis and cystic degeneration(P=0.001), multiple nodule fusion(P=0.000), plain T1WI signal(P=0.015), and associated metastasis(P=0.000). There were no statistically significant differences in tumor size(F=0.363; P=0.698), tumor shape(P=0.277), boundary(P=0.221), lengthwise growth(P=0.401), plain scan T2WI signal(P=0.835), intensifying degree(P=0.338), whether the tumor had capsule(P=0.423).@*Conclusion@#The onset age of benign GN is significantly larger than that of NB, T1WI plain scan is mostly low signal, after enhancement, mainly with mild strengthening, GNB and NB tumor shape is irregular, NB is mainly multi nodular fusion, which easy to occur hemorrhage and necrosis cystic change, T2WI is mainly mixed high signal, mostly moderate or clear uneven enhancement, NB is prone to invasion of the spinal canal and distant metastasis.

ABSTRACT

Objective Pulmonary function testing is a commonly used indicator for clinical evaluation of the degree of pulmonary fibrosis in patients. This paper aims to investigate the dynamic changes of lung function in mice with pulmonary fibrosis and to establish a range of reference values for lung function parameters in normal Kunming mice. Methods Twenty-eight SPF Kunming mice were randomly divided into normal control group (n=14) and model group (n=14). After anesthesia with 10% chloral hydrate, the normal control group only underwent tracheal puncture. The model group received intratracheal puncture and injection of bleomycin (BLM) (5 mg/kg body weight), and the lung function indicators of all mice were detected in the same order on the 1st, 2nd, 3rd and 4th weekends after modeling: Ti, Te, PIF, PEF, TV, EV, RT, MV, f, Penh and EF50. Results After intratracheal BLM injection, mice in the model group showed decreased hair softness and smoothness, hair loss and decreased activity after the 2nd week. Compared with the control group, Ti, Te and RT values in the model group significantly increased at week 4 (P<0.05), while the values of PEF, RT, MV, f and EF50 decreased significantly at the same week (P<0.05). Compared with the model group at week 1, the differences in Ti, Te, RT and f values at week 2, 3 and 4 were statistically significant (P<0.05); the differences in MV and EF50 values at week 3 and 4 were statistically significant (P<0.05); while the PIF values only showed differences at week 4 (P<0.001). Compared with the Penh values in the control group at week 2, 3 and 4 (0.553±0.189, 0.662±0.164, 0.712±0.189), the differences of the model group (0.820±0.205, 0.936±0.188, 1.053±0.236) showed statistical significance (P<0.001). Compared with the model group at week 1, the differences of Penh values in the model group only showed statistical significance at week 3 and 4 (P<0.05). Through four-week lung function test, various parameters were obtained, among which the normal range of the main index Penh value was 0.27-0.88. Conclusion The lung function detected by the non-invasive whole body plethysmography system was stable and reliable with good effects; the lung function in mice with the BLM-induced pulmonary fibrosis continued to decrease within four weeks. Penh, which reflects airway resistance, can be used for overall screening of the lung function among the test mice after two weeks of modeling.

OBJECTIVE@#We aimed to explore how fermented barley extracts with Lactobacillus plantarum dy-1 (LFBE) affected the browning in adipocytes and obese rats.@*METHODS@#In vitro, 3T3-L1 cells were induced by LFBE, raw barley extraction (RBE) and polyphenol compounds (PC) from LFBE to evaluate the adipocyte differentiation. In vivo, obese SD rats induced by high fat diet (HFD) were randomly divided into three groups treated with oral gavage: (a) normal control diet with distilled water, (b) HFD with distilled water, (c) HFD with 800 mg LFBE/kg body weight (bw).@*RESULTS@#In vitro, LFBE and the PC in the extraction significantly inhibited adipogenesis and potentiated browning of 3T3-L1 preadipocytes, rather than RBE. In vivo, we observed remarkable decreases in the body weight, serum lipid levels, white adipose tissue (WAT) weights and cell sizes of brown adipose tissues (BAT) in the LFBE group after 10 weeks. LFBE group could gain more mass of interscapular BAT (IBAT) and promote the dehydrogenase activity in the mitochondria. And LFBE may potentiate process of the IBAT thermogenesis and epididymis adipose tissue (EAT) browning via activating the uncoupling protein 1 (UCP1)-dependent mechanism to suppress the obesity.@*CONCLUSION@#These results demonstrated that LFBE decreased obesity partly by increasing the BAT mass and the energy expenditure by activating BAT thermogenesis and WAT browning in a UCP1-dependent mechanism.

ABSTRACT

Objective To investigate the clinicopathological and magnetic resonance imaging (MRI)characteristics of mediastinal neuroblastoma (NB) in children,and to improve the preoperative diagnosis accuracy of NB.Methods The clinicopathological characteristics and imaging data of 16 patients with pediatric NB confirmed by pathology in Children's Hospital of Chongqing Medical University from January 2012 to December 2016 were retrospectively analyzed,including the onset age,lesion size,shape and boundary of the tumors,and MRI scan,and the characteristics of the enhanced signals,which were compared with the postoperative pathology,then the data were also analyzed.Results The age of onset was from 30 d to 11 years old,with the median age of 1.98 years old;all cases occurred in the posterior mediastinum,including 7 cases in left superior posterior mediastinum,4 cases in right superior posterior mediastinum,3 cases in left posterior mediastinum,1 case in right posterior mediastinum,1 case in right posterior mediastinum,1 case in neck and 2 cases in adrenal area;the maximum diameter of tumor was 3-10 cm,with the average of 6.4 cm;the tumor morphology and boundary:10 cases were irregular and 6 cases were regular;I0 cases had multiple nodular fusion,12 cases were lengthwise,12 cases had clear border,1 case had crossing the midline of the tumor,2 cases had multiple tumors,and 15 cases had tumor and spinal canal.MRI signal:plain scan T1WI showed low signals in 4 cases,equal signals in 12 cases.Plain scan T2WI showed uniform high signals in 9 cases,mixed high signals in 7 cases;hemorrhagic necrotic cysts of tumors in 11 cases,slight enhancement in 2 cases,moderate enhancement in 5 cases,and strengthened enhancement in 5 cases.Postoperative pathology showed that there were 9 cases of complete capsule and 5 cases of incomplete capsule or without capsule,including,2 cases of surrounding organs invasion;and 7 cases transferred from other parts,including 5 cases of bone metastasis,2 cases of bone marrow,2 cases of lymph nodes,1 case of left lung,and 1 case of bone and soft tissue.The cut surface of the tumors showed gray in 4 cases,grey brown in 6 cases,gray white and brown in 4 cases;hemorrhage in 2 cases,calcification in 2 cases,necrosis in 3 cases,cystic degeneration in 1 case,hemorrhage and necrosis in 2 cases,necrotic calcification in 2 cases,and hemorrhagic calcification in 2 cases.There were 4 cases combined with pleural effusion,and 4 cases combined with pneumonia,4 cases of Horner syndrome,and 1 case of lung consolidation or atelectasis.Conclusion The MRI manifestations of pediatric NB patients have some characteristics.Combination with their clinical characteristics can provide an important basis for the early diagnosis and accurate diagnosis of NB.

ABSTRACT

Objective To investigate the effect of recombinant botulinum neurotoxin serotype A heavy chain (BoNT/A heavy chain)on local proteins which are related to nerve growth after spinal cord injury in rats,and to get some experimental evidence to explain the mechanism of BoNT/A heavy chain in stimulating neuritogenesis. Methods Recombinant botulinum neurotoxin serotype A heavy chain was applied locally or intrathecally to rats with ipsilateral semi-dissociated lumbar spinal injury. Local spinal tissue was extracted for general protein expression by two dimension electrophoresis plus nitrate silver staining after different time period of injury. Based on the results of 2-D gel electrophoresis,growth-associated protein 43(GAP-43)and of superior cervical ganglion 10(SCG 10)were selected to examine the changes of their expression and distribution features under BoNT/A heavy chain administration using SDS-PAGE,western blot and immunofluorescence. Results (1)The model of spinal cord injury(SCI)in this study was an ipsilateral semi-dissociated lumbar SCI in rat. The rats showed obvious motor and sensory dysfunction in the ipsilateral hind limb.(2)The results from 2-D gel electrophoresis plus nitrate silver staining showed that the administration of BoNT/A heavy chain based on SCI altered the local protein expression pattern. The decrease or increase in the expression of some protein dots /dots group was clearly seen after single SCI. However, these changes were transformed by BoNT/A heavy chain treatment,which appeared as a reversed pattern turning toward that in control group or further increased expression upon SCI,such as the dots located respectively at 35-45 kDa and 18-25 kDa level,pI between 5-7. In addition,the expression of the two dots located at the level as above increased after SCI only, and showed further increase in their expression with BoNT/A heavy chain intervention.(3)The changes of selective GAP-43 and SCG 10 expression and distribution by western blot and immunofluorescence indicated that the administration of BONT/A heavy chain based on SCI amplified the expression of GAP-43 and SCG 10(P < 0.05). Meanwhile,the positive immuonfluorescent staining for both GAP-43 and SCG 10 mainly distributed nearby the proximal area of injury, both cytoplasm and neuronal processes were positively stained. Conclusions Intrathecal delivery of BoNT/A heavy chain increases the expression of growth-associated proteins GAP 43 and SCG 10 after SCI in rats.

ABSTRACT

Objective:To improve the quality standard for Jinhuanglian oral liquid. Methods:The three herbs in the preparation, Honeysuckle, Scutellaria Baicalensis Georgir and St John's Wort, were identified by TLC qualitatively. The content of chlorogenic acid in Lonicerae Japonicae Flos was determined by HPLC. The chromatographic separation was carried out on a Promosil C18 (250 mm × 4. 6 mm,5 μm) column at 25℃. The mobile phase consisted of acetonitrile-0. 4% phosphoric acid (10 :90) with gradient elution at a flow rate of 1. 0 ml·min -1 , and the injection volume was 10μl. The detection wavelength was 327 nm. Results:The spots in TLC were clear without any interference. The linear range of chlorogenic acid was 5.325-170.400 μg·ml-1(r=0.9998). The average recovery was 97. 95% and the RSD was 2. 04%(n=6). Conclusion:The method is reliable and easy to operate,which can be used as the quality control method for Jinhuanglian oral liquid.

Objective@#To observe the effect of the expressive or functional blockage of TRPV1 on nerve regeneration after sciatic trans-section injury.@*Methods@#AMG-517, a kind of TRPV1 inhibitor, was injected into the surrounding area of the ipsilateral lumbar dorsal root ganglia while unilateral sciatic nerve was transected. A total of 24 healthy male Sprague-Dawley rats were divided into 4 groups: control group, injury only group, injury+ AMG-517 150 μg/kg group, injury+ AMG-517 300 μg/kg group. The injury only group was injected the same volume of medium. The release of CGRP from dorsal-horn of spinal cord, the number of axons at proximal stem of sciatic nerve after transection, and the expression of TRPV1 in dorsal root ganglion were detected using the methods of ELISA, Western blot and semi-thin section (1 μm)- toluidine blue staining 2 weeks after injury.@*Results@#The release of CGRP in lumbar spinal dorsal horn was obviously decreased after AMG-517 treatment, which was the evidence of TRPV1 functional inhibition. CGRP in the control group was 0.15 ng/g, the injury only group 0.17 ng/g, AMG-517 150 μg/kg group 0.09 ng/g, and AMG-517 300 μg/kg group 0.11 ng/g(P<0.01). The number of axons which were myelinated or unmyelinated increased after the TRPV1 was inhibited by AMG-517(P<0.01). In addition, the injection of AMG-517 into surrounding dorsal root ganglion decreased the expression of TRPV1 in dorsal root ganglion(P<0.01).@*Conclusions@#Over expression or activation of TRPV1 after periphery nerve injury has negative effect on nerve regeneration in fact; Inhibiting the over-expression or over-activation of TRPV1 after nerve injury facilitates axonal regeneration and nerve repair.

ABSTRACT

Objective The active ingredient was used as index to optimize the extraction and enrichment process of anti-in-flammatory and analgesic active-fraction(ARF)of Dianbaizhu. Methods Methyl salicylate triglycoside-B was chosen as index com-ponent to extract and enrich methyl salicylate glycosides. Extraction and elution solvents were optimized. The HPLC fingerprint was ob-tained with Thermo Hypersil Gold C18(250 mm×4.6 mm,5μm)column and a gradient elution with the mobile phase consisting of ace-tonitrile(A)-0.2%acetic acid(B)at a flow rate of 1.0 ml/min. And the detection wavelength was set at 294 nm. Results The opti-mized extraction solvent of Dianbaizhu was the 30%ethanol and the optimized elution solvent of ARF enriched by AB-8 macroporous resins was the 35%ethanol. The methodological study on similarity and RSD in ARF HPLC fingerprint of three batches of samples cor-responded to related regulations. Conclusion The extraction and enrichment process of ARF is stable and repeatable.

This study explored how bitter melon powder (BMP) alters the colonic microenvironment during the development of obesity-associated fatty liver in rats. We observed that BMP effectively inhibited the body weight gain and lipid accumulation in the liver, ameliorated glucose intolerance, and increased the colon weight after an 8-week treatment compared to that in the high-fat diet (HFD) group. BMP significantly decreased fecal water toxicity towards HT-29 cells, as revealed by the cell counting kit (CCK)-8 assay results, and the mRNA expression of Toll-like receptor 4 (TLR4) in colon mucosa. Additionally, gut permeability in the BMP group was restored to normal levels. Finally, BMP alleviated the inflammatory state of the rat colon mucosa and liver tissues as well as the systemic inflammation.

ABSTRACT

Objective:To improve the quality standard for Jinhuanglian oral liquid. Methods:The three herbs in the preparation, Honeysuckle, Scutellaria Baicalensis Georgir and St John's Wort, were identified by TLC qualitatively. The content of chlorogenic acid in Lonicerae Japonicae Flos was determined by HPLC. The chromatographic separation was carried out on a Promosil C18 (250 mm × 4. 6 mm,5 μm) column at 25℃. The mobile phase consisted of acetonitrile-0. 4% phosphoric acid (10 :90) with gradient elution at a flow rate of 1. 0 ml·min -1 , and the injection volume was 10μl. The detection wavelength was 327 nm. Results:The spots in TLC were clear without any interference. The linear range of chlorogenic acid was 5.325-170.400 μg·ml-1(r=0.9998). The average recovery was 97. 95% and the RSD was 2. 04%(n=6). Conclusion:The method is reliable and easy to operate,which can be used as the quality control method for Jinhuanglian oral liquid.

ABSTRACT

AIM:To observe the effect of botulinum neurotoxin type A heavy chain ( BoNT/A HC) on the pat-tern of spinal protein expression by intrathecal injection after spinal cord injury in rats , and to explore the role of BoNT/A HC intervention in spinal protein expression and some of its mechanisms in nerve regeneration after injury .METHODS:The model of unilateral lumbar spinal cord injury was established .The effects of BoNT/A HC intervention at different doses (2 μg, 4 μg, 6 μg and 8 μg) on the general pattern of protein expression in the spinal cord tissues at the injury site and the cranial part adjacent to the injury site was measured and evaluated by SDS-PAGE and Coomassie brilliant blue staining first, and then by two-dimensional SDS-PAGE.RESULTS:The histological structure of the ipsilateral side of lumbar spi-nal cord showed obvious destruction and degradation , mainly affecting both gray and white matter of the left side of the cord.The result of SDS-PAGE with Coomassie brilliant blue staining from injured spinal cord tissue displayed that the ex-pression of some proteins after one-time BoNT/A HC treatment appeared obviously different from that without BoNT /A HC treatment.Moreover, the pattern of the protein expression affected by BoNT/A HC was similar to that of the normal spinal cord.The more detail information from two-dimensional SDS-PAGE indicated that more than 10 proteins with different mo-lecular weight and isoelectronic points were differentially expressed at day 2 and day 20 after local injection of 6μg BoNT/A HC.This altered expression actually appeared a tendency toward the pattern shown in normal group .CONCLUSION:The immediate application of BoNT/A HC at the injury site after unilateral lumbar spinal cord injury is able to affect the pattern of local protein expression .The altered protein expression by injury could be reversed back to normal or approxi -mately normal by local BoNT/A HC administration.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the features of immunophenotypes and the characteristics of molecular biology and cellular genetics of AML patients with CD7 and CD4 expression.</p><p><b>METHODS</b>The immunophenotypical markers of AML cells were detected by multiple parameter flow cytometry; the expression of WT1, MDK, ETO, PML-RaRa and BCR-ABL were detected by RT-PCR; and cellular features were analyzed by R-band in 304 patients. The patients were divided into three groups according to their immunophenotypes: AML with CD7 expression (CD7 group), AML with CD4 expression(CD4 group) and AML without CD7 and CD4 expression (common AML group).</p><p><b>RESULTS</b>The expression rate and level of HLA-DR in CD7 group were higher than those in the common AML group, and the expression rate of CD33 and CD34 was higher than that in the other two groups. The expression rate and level of CD15, CD64 in the CD4 group were higher than those in the other 2 groups, and the expression rate and level of CD33 were higher than those in the common AML group. WT1 expression in the CD7 group was lower than that in the common AML group. PML-RaRa was not detected in the CD7 group. AML with co-expression of CD4 or CD7 showed more normal karyotype. (15;17) was not found in AML with CD7 expression.</p><p><b>CONCLUSION</b>AML cells with CD7 expression originate from precursor cells and are blocked in the early phase of hematological development; AML cells with CD4 expression originate from more mature stage of hematological devevelopment and with CD33, CD64 and CD15 high expression; AML cells with CD7 and CD4 expression are characterized by no-specific change of cellular genetics. According to the expression level and intesity of CD4 and CD7, and together with other specific lineage markers, the MRD in AML patients can be quantitatively detected.</p>

ABSTRACT

Objective:To provide some thoughts for pharmaceutical treatment and care for the patients with pulmonary infection af-ter renal transplantation. Methods:Clinical pharmacists participated the whole treatment process of one case of pulmonary infection af-ter renal transplantation. According to the literatures combined with medical history, clinical symptoms and lab results, the drug treat-ment process of the patient was analyzed, and the key points of the optimized pharmaceutical care were summarized. Results: The pharmaceutical care included the dose adjustment of immunosuppressants at the early phase of the disease and after the improvement of clinical symptoms, attention paid to the interactions between multiple anti-infective drugs and immunosuppressive agents, dosage ad-justment based on the renal function of the patient, monitoring adverse drug reactions and drawing up personalized regimen. Conclu-sion:Through comprehensive medication monitoring, clinical pharmacists can help physicians develop timely and effective treatment programs and provide professional and effective pharmaceutical care for patients.

ABSTRACT

Porcine reproductive and respiratory syndrome virus (PRRSV) and porcine circovirus type 2 (PCV2) are very two important pathogens that have coursed huge economic losses in swine production in worldwide. In this study,a vector pCMV-TJM containing the full-length cDNA clone of PRRSV attenuated strain TJM-F92 was firstly constructed by PCR method. Then a gene sequence containing Afl II/Mlu I e restriction enzyme sites and a transcription regulatory sequence for ORF6 (TRS6) was inserted be- tween ORF7 and 3'UTR, yielding a expression vector pCMV-TJM-TRS. Subsequently, a plasmid pCMV-TJM-Cap was constructed by cloning of PCV2 ORF2 gene into the unique sites Afl II /Mlu I of pCMV- TJM-TRS plasmid DNA. Then three recombinant PRRSV, rTJM, rTJM/TRS and rTJM/Cap, were rescued by transfection of pCMV-TJM, pCMV-TJM-TRS and pCMV-TJM-Cap into Marc-145 cells, respectively,and confirmed by the genome sequence, restriction enzyme digestion, Western Blot and IFA. They all had the molecular markers which was different from the parent virus. The growth characteristics of the rescued viruses were similar to that of parent virus. rTJM/Cap could also express efficiently PCV2 Cap protein in Marc-145 cells. At passage 8, it still had PCV2 ORF2 gene which examined by RT-PCR. It indicated that the full-length cDNA clone of PRRSV attenuated strain TJM-F92 and recombinant PRRSV rTJM/Cap expressing PCV2 Cap protein were successfully constructed. It made an important foundation for studying on the pathogenic mechanisms of PRRSV and PRRSV-PCV2 vaccine in the future.

ABSTRACT

Objective:To analyze the use status of antibiotics and the resistance of clinic isolate bacteria against the commonly used antibiotics before and after the intervention. Methods:Using the information retrieval systems, the consumption of antibiotics in the inpatients during the 1st quarter of 2012(before the intervention) and the same period of 2013(after the intervention) was com-pared. According to the defined daily dose ( DDDs) , the antibiotics were ranked, and the resistance rate against the commonly used antibiotics was analyzed. Results:Compared with the top ten before the intervention, the top ten after the intervention was changed sig-nificantly, however, cephalosporins was still the main species. After the intervention, the overall decline in DDDs was significant, the separation rate and distribution of bacteria remained stable. ESBLs enzyme production rate of Enterobacteriaceae Escherichia coli was re-duced by 7. 61%, and that of Klebsiella pneumoniae was reduced by 1. 34%, and the resistance rate against the commonly used antibi-otics was in an overall downward trend. The resistance rate of Gram-positive staphylococci against the commonly used antibiotics was de-creased, while that of Gram-positive enterococci showed notable difference. Conclusion:The DDDs of antibiotics and bacterial resist-ance rate are in an overall downward trend in our hospital after the intervention;however, there is still exception. Therefore, the clini-cal antimicrobial susceptibility tests should be performed as soon as possible to help the choice of antibiotics.
